Member Services
ShareCare offers services to Leelanau County seniors, 60 years and older, regardless of their ability to pay. The annual enrollment fee is $90 per individual. These fees can be waived due to financial hardship. As a nonprofit organization, we do rely on donations to fulfill our mission. If you can donate above the enrollment fee, we appreciate your contribution. All donations are tax-deductible.

Intake Questionnaire
Once you've filled out the form, save it to your computer. You can either print the completed form and mail it to: ShareCare, PO Box 157, Suttons Bay, MI 49682, or you can email it to: info@sharecareleelanau.org. Prefer to talk to someone? Call the ShareCare office at 231-256-0221 to have your questions answered, or to schedule an appointment with a member of our Board of Directors to meet with you in your home.
